Boulevard Rebate
Eligibility Requirements:
- Purchase the Boulevard software program between 1/1 - 1/31/2012
- Submit Rebate Form along with
- Proof of purchase from a Mary Kay® specific software or web service
Proof of purchase may include:
- An invoice for payment made for software or web services purchased
- A credit card statement showing charge from an applicable company
- The database from the MK specific software program you are using
Proof of purchase must be submitted in conjunction with the rebate form by March 1, 2012.
